Together Toward Transplant Program

Off The List was founded on the belief that you and your family deserve support and access to tools that will enable you to “get off the list” and on to living your best life.  We believe the best way to do this is by partnering together through our Together Toward Transplant program.

Our 12 week, step-by-step program provides you personalized and emotional support as well as assistance in developing a customized marketing campaign to help you find a donor.

As a participant in Together Toward Transplant, you will have access to Kidney Transplant 101 Guide and How to Find Your Living Kidney Donor: Your Step-By-Step Guide. In addition, you will participate in the following programs:

    • Transplant PalsTM  1:1 mentorship program that pairs you with an individual who is post-transplant and has successfully navigated the transplant process.  You will have the opportunity to build a relationship with someone who has personally walked the transplant journey and understands how you are feeling.  Your Transplant Pal is available to answer questions and share their experience and knowledge with you and your family.  You can expect to connect with your Transplant Pal multiple times a month through phone calls, emails, texts and in-person meetings.
    • Transplant CirclesTM: Support groups where you can connect with other individuals going through the transplant process. Your Transplant Circle provides you a safe place to talk and share with others who understand what you are going through.  These groups meet once a month either in-person or virtually.
    • Transplanting HopeTM:  Ongoing encouragement for you and your family through regular phone calls, cards, and weekly emails from one of our Transplant Journey Professionals who understand the transplant process. You are not alone…we will walk through this journey with you. And don’t worry, even after you have completed the three-month program, we are still here to help you navigate your kidney journey and support your efforts in finding a living donor.

While sitting at an event to learn more about how to find a donor for my husband, I felt like a big failure. I was and am my husband’s kidney champion. It was my job to find him a living donor and quick. At that meeting, all the raw emotions of despair that were deep inside bubbled to the top, I was on the ledge. Then, a woman named Darci walked across the room and with her infectious personality showed me kindness, validation, compassion, and gave me a plan to march forward in the quest of finding a donor. Throughout our entire kidney journey and beyond, she has cheered us forward and always in the right direction. Words of thanks would never be able to express my gratitude to this amazing woman.
